Instructions for Online Nephrology Consult Letter Template

The link below will open in a new browser window that will contain the Nephrology Consult Letter Template. Directions will appear in red or inside form windows throughout the document.

Using the drop down menus, fill-in windows, and check boxes, fill in all requested information. When you have finished entering information, click on the button at the bottom that says "Create Letter." This will NOT send the letter or any data to anyone; instead, it will create a letter as a web page that you can then save as a document for editing in your word processor, or send directly to your printer.
