Instructions for Downloadable Prototype Template

Downloading
To download the prototype Template:

• Click on the link below
• Save the Template to your computer by clicking on the save icon located in the upper left hand corner of the Adobe Acrobat Reader toolbar
• Select your desktop or other desired location
• Click <Save>

Next, open the Template file and use the drop down menus, fill-in windows, and check boxes to enter all requested information on pages 1 through 5.

Printing
Once all of the information has been entered, you must click on the "Print Letter" button on page 6 to print the letter to your printer. You can then review the hard copy, make any necessary changes, and reprint the final letter, again using the "Print Letter" button. (Please note: using normal print commands such as <File and Print>, <Control and P>, or pressing the printer icon will produce errors in formatting.)

Saving Final Letters
The patient information entered will not be stored or saved in this Template or on your computer once the application is closed. You will need to enter all data, generate the letter, and print it out before exiting the program. If you would like to save a copy of the final letter, you will need to copy and paste it into a word processing application, such as MS Word or WordPerfect. (To copy the letter, first click on the Text Select Tool on the Adobe Acrobat Reader toolbar, highlight the text, copy it and paste the letter directly into your document.)
